You may use the form below to submit a complaint to the Office of Education Accountability.  Please note that the information provided will be kept confidential.  If you choose to provide contact information, it will be helpful for OEA staff as we consider the information in your complaint.  We will not divulge your identity to anyone unless you specifically authorize us to do so.  You may choose to remain anonymous if you desire to do so.  This choice will not affect the manner in which your complaint is handled by the OEA.
