Last Effective Date

  • The Last Effective Date Excel file is for informational purposes only.
  • It is the agency's responsibility to either complete the amendment process or file a certification letter prior to a regulation's expiration. If a discrepancy with a last effective date exists, please notify The Regulations Office will answer these types of inquiries as workload permits. If date is in question, it is still agency's responsibility to file a certification letter prior to expiration. If in doubt, to help avoid expiration use the earliest date.
  • Regulations staff is not able to assist with MS Excel questions. For MS Excel help, please contact your technology administrators or perform an Internet or Youtube search.

Certification Letter Templates

  • The certification letter form is available in two different formats: PDF and MS Word.
  • The letter must be on letterhead and may be filed with the Regulations Compiler either in hard copy format or by email.
  • Because a letter is needed for every file, due to space limitations in the Compiler's office a separate letter is needed for each administrative regulation.
