901 KAR 5:060. Birth certificate reissued; circumstances.

      STATUTORY AUTHORITY: KRS 194.050, 211.090

      NECESSITY, FUNCTION, AND CONFORMITY: KRS Chapter 213 relating to Vital Statistics authorizes the Cabinet for Human Resources to regulate the registration of births in Kentucky. The purpose of this administrative regulation is to provide a uniform procedure for issuing a new birth certificate where the old certificate is mutilated, defaced or contains information of a personal sensitive nature which might embarrass the applicant and which is not pertinent to the vital information required in the certificate.

 

      Section 1. Issuance of New Birth Certificates Under Certain Circumstances. The Director of Vital Statistics may issue a new birth certificate where the original certificate is mutilated, defaced or contains information of a personal sensitive nature which might embarrass the applicant and which is not pertinent to the vital information required in the certificate.
